Palliative Care

As part of LWHA's commitment to best practices and providing the best possible care to the people we serve, we are supporting nurses to become champions of palliative care. Nurses complete a 6 week prerequisite in Fundamentals of Hospice Palliative Care followed by a 6 month program called Comprehensive Advanced Palliative Care Education (CAPCE). The nurses are specially trained and prepared to work with a palliative patient to assess their needs, create an individualized patient care plan and make appropriate recommendations to the physician. Our nurses work in partnership with our physicians, the Huron-Perth Palliative Pain & Symptom Management Program Consultant, the Huron Hospice Volunteer Service (Wingham site) and North Perth Community Hospice (Listowel site).CAPCE – Comprehensive Advanced Palliative Care Education.

The physicians and staff of LWHA request that patients bring all current medications to the hospital for Emergency Room visits, preoperative clinics and planned admissions.  To provide the best possible medication care the health care team must know about all medications taken.  This includes drugs taken by prescription and medicines purchased over the counter. For more information on Medication Reconciliation click here.
