Complex Continuing Care

The Complex Continuing Care (CCC) unit provides supportive care for chronically ill patients as well as those awaiting placement in long term care facilities.  CCC provides 24-hour nursing care and access to physiotherapy, occupational therapy, speech language pathology or dietitian services for those in need by a doctor’s referral (see Therapy section for more information).  Nutritious meals are offered following a rotational menu cycle (see Patient Services section for more information).
Medications are managed under the care of a physician and clinical pharmacist. The physicians and staff of LWHA request that patients bring all current medications to the hospital for Emergency Room visits, preoperative clinics and planned admissions.  To provide the best possible medication care the health care team must know about all medications taken.  This includes drugs taken by prescription and medicines purchased over the counter.  On occasion patients may have to supply their own medications such as birth control tablets, eye drops, nasal sprays, vitamins and herbal products.  For more information on Medication Reconciliation click here.
Discharge planning and coordination is completed by Community Care Access Centre (CCAC) Case Managers.   The Case Managers will help you and your family / power of attorney etc to make the best informed decision for your most appropriate care or placement, following discharge.

A chronic co-payment fee may be applicable in some situations such as a long term care placement or long term care required in hospital.  This will be discussed by the CCAC Case Manager and hospital business office personnel.
